Our Case Manager plays a pivotal role in ensuring that each veteran and first responder receives tailor passionate support both during and after their participation in our programs.

Personalized Support Plans

  • Initial Assessment
    Each veteran's and first responder's unique needs, goals, and challenges are evaluated to create a customized plan.​​

  • Resource Navigation

        Guidance on accessing benefits, healthcare services, mental health resources, and employment opportunities.

 

Advocacy and Assistance

​

  • Veteran Advocacy

       Acting as a liaison to ensure veterans receive the benefits and support, they're entitled to.​

​

  • Crisis Support

​        Immediate intervention and referrals for veterans and first responders facing mental health or financial crises.

​

​Continuous Engagement

​

  • Follow-Up Services

        Regular check-ins to monitor progress, address new challenges, and adjust plans as needed.

 

Peer Support Networks

​

  • Facilitating connections with other veterans/first responders and support groups for continued camaraderie.

​

Assistance in Obtaining and Coordinating with Other Public Benefits

​

  • Healthcare and daily living services

​        Identify eligibility for VA options or other health care resources.​

​

  • Personal Financial Planning and Credit Counseling

       Help with day-to-day finance and help achieve a term budget and financial goals.

 

Temporary Income Support Services for Veterans

​

  • Including help with mental health benefits, food assistance, housing assistance, employment counseling, medical assistance, VA benefits, and income support assistance.

 

Job Placement Support​

​

  • Assistance with resume building, job applications, and career counseling.

 

Skill-Building Workshops​

​

  • Training sessions focused on leadership, communication, and problem-solving.

Holistic Wellness Guidance​​

​

  • Mental and Physical Health Referral

        Connecting veterans and first responders to therapists, counselors, and wellness practitioners.​​​

  • Lifestyle Coaching

        Encouragement for healthy living, stress management, and personal growth.
